A few decades ago, the Institute for Magical Study used their knowledge of teleportation magics to create permanent circles in major cities, and maintained a transport network that funded their research. The Institute is gone now, but the Circle has stepped into the void. The cost of paying a specialist, magical components, and the brief time a portal can be maintained mean that it’s only cost-effective for finished luxury goods and wealthy people; anything else that needs to be transported goes overland. Some people are afraid of portal travel, due to the rare accidents where someone loses a limb (if they’re lucky), but with orcs, giant scorpions, and unfriendly desert spirits between here and Juarez or Independence, it really is the safest way to travel.
